Why do I need a primary care physician?

Catholic Health’s primary care physicians will be your point of contact for your everyday health needs. We will make sure you understand all the elements of your care plan, including why we are recommending certain medications, treatments or health screenings. Your primary care physician can help prevent or treat diseases early, and can spot patterns or changes to your overall health.

All practices welcome new patients and accept most insurance plans. Extended hours, next-day and same-day appointments are often available. Catholic Health also offers:

  • Convenient access to primary care and specialty care providers across Long Island
  • Easy online appointment scheduling and video visits
  • 6 full-service hospitals, with emergency services
A PCP treats conditions such as:

What is preventive care?

Preventive care helps you stay healthy by preventing and catching conditions like diabetes and heart disease early. We perform preventive screenings for certain conditions if we suspect you are at risk of developing them. Additionally, your primary care physician will help coordinate your routine health screenings based on your age or determined risk level.

What if I need a specialist or other services?

Our primary care physicians seamlessly and quickly coordinate your care. They can provide referrals to specialists, radiology centers, physical therapists and other health services within Catholic Health whenever you need them.

Your primary care provider will arrange for urgent care or make a referral to Catholic Health’s Patient Navigation Hub for all other care.

What can I expect on my first visit with a Primary Care Physician?

On your first visit to a Catholic Health primary care provider, you will talk about your personal and family health history. A nurse will take your temperature, blood pressure and pulse as well as measure your height and weight. Your physician will perform a physical exam and talk with you about your health. This visit is a great time to discuss your personal health goals.

  • How do you feel about your overall health?
  • What is your level of physical activity?
  • In order, what are your top three major health concerns?
  • What is your personal health goal and what have you done so far to achieve it?
